What to Bring to Your Visit

 

Always bring a valid photo ID, copy of your active insurance card (primary and secondary). Medicare cards must be the newly issued 2020 cards.

If you are a minor under the age of 18, you must be accompanied by a parent or legal guardian.

Self Pay or INsurance

 
 

We are always available for both non-insured and insured patients. We are in-network with the plans below.

Please check with your insurance provider to ensure we are under your exact plan or if you can visit us under your out-of-network benefits. All deductible and copay/coinsurance are due at the time service is rendered. Please call your insurance plan if you do not understand your benefits as no plan under the same carrier is alike.
